Affordable Connectivity Program

To apply your Affordable Connectivity Program ("ACP") benefit to your CenturyLink or Quantum Fiber account, please complete this form. You also may transfer your existing benefit from another provider to CenturyLink or Quantum Fiber by completing this form.

For CenturyLink to apply the ACP benefit to your account or to switch your existing CenturyLink Internet plan to the CenturyLink Affordable Connectivity Program Internet plan ("ACP Internet Plan"), you must complete the form below. First, please complete the following steps:
  1. Apply or recertify for the Affordable Connectivity program at https://ACPbenefit.org/. Please use the information for the primary CenturyLink account holder when completing the program application.
  2. Receive your benefit eligibility confirmation from the National Verifier.
  3. If you are not currently a CenturyLink or Quantum Fiber customer, please sign up for Internet service first.

How do I find my Account Number?

CenturyLink Customers: You can find your account number on the first page of your CenturyLink bill in the very top row. If you don't have a paper bill handy, you can try retrieving your account number via email address.
Quantum Fiber Customers: Please leave the account number field in the form below blank.

The Affordable Connectivity Program is a Federal Communications Commission benefit program of the federal government that reduces your broadband Internet service bill.
You are not required to sign up for ACP-supported service to obtain or continue to obtain Lifeline-supported services. Qualifying consumers can participate in both the ACP and the Lifeline program, and may apply their ACP benefit to their Lifeline service or to a different service. To sign up for, renew, upgrade, or modify Lifeline services with CenturyLink or Quantum Fiber, please visit: https://www.centurylink.com/aboutus/community/community-development/lifeline.html.
You may obtain broadband Internet service supported by the Affordable Connectivity Program from any participating provider of your choosing, you may apply the affordable connectivity benefit to any of our broadband service offerings at the same terms available to households that are not eligible for Affordable Connectivity Program-supported service, and you may transfer your benefit to another provider at any time.
Enrollment Disclosures.If you transfer your benefit to another provider, but continue to receive service from CenturyLink/Quantum Fiber, or if you are de-enrolled from the Affordable Connectivity Program, you will be subject to the CenturyLink or Quantum Fiber's undiscounted rates and general terms and conditions. CenturyLink or Quantum Fiber may disconnect your Affordable Connectivity Program-supported broadband internet service after 90 consecutive days of non-payment.
Transfer Disclosures.You are limited to one ACP transfer each service month, with limited exceptions. If you are transferring your ACP benefit from another service provider to CenturyLink/Quantum Fiber, the effect of the transfer will be that the ACP benefit will be applied to the CenturyLink/Quantum Fiber service, not the service retained from your previous provider. If you choose to maintain service from your previous service provider, your previous service provider may charge you their undiscounted rates for that internet service.
You may contact CenturyLink or Quantum Fiber customer centers, as applicable, for help with ACP questions. If you have a complaint about your ACP service, you may file it via the FCC's Consumer Complaint Center.
Please apply the ACP discount to my CenturyLink/Quantum Fiber Internet plan. I was provided enrollment and transfer disclosures and, after receiving these disclosures, consent to having my ACP discount applied to CenturyLink/Quantum Fiber Internet service.
Future Affordable Connectivity Program Planning
If the Affordable Connectivity Program you are enrolled in ends, you can choose to continue to receive Internet service, subject to the generally applicable rates, terms, and conditions for your service, or you can choose to have your Internet service end automatically.
Consent to Transmit Information to the Affordable Connectivity Program Administrator
To enroll you in the Affordable Connectivity Program, we must transmit the following information about your enrollment to the administrator of the Program, the Universal Service Administrative Company (USAC) to ensure the proper administration of the program: your full name, full address, date of birth, last four digits of your Social Security Number (if applicable), and telephone number associated with your enrollment along with the same information (e.g. name, address date of birth, last four of their Social Security Number) of your minor child or dependent if your child or dependent is the basis for qualifying for benefits under the program. If you consent to us transmitting this information, please indicate so by checking below. If you do not consent, we will not be able to enroll you in the program and you will not receive the benefit.
