GENERAL TERMS: Offers and rates apply to business customers only. The following sections apply to all business services: General, Terms and Conditions, Taxes, Fees and Surcharges, Monthly Rate, and Discounts and Credits.

GENERAL: Services and offers not available everywhere. CenturyLink may change or cancel services, or substitute similar services at its sole discretion without notice. Requires credit approval and deposit may be required. Additional restrictions apply.

TERMS AND CONDITIONS: All products and services are governed by tariffs, terms of service, or terms and conditions posted at http://www.centurylink.com/Pages/AboutUs/Legal/ ("Website"). Products and services are provided by the CenturyLink operating company serving your service location.

TAXES, FEES, AND SURCHARGES: All prices exclude taxes, fees and surcharges, which apply to all services, and include federal and state-mandated and permitted charges, cost recovery charges, state and local fees that vary by area, and certain in-state surcharges. Cost recovery fees are not taxes or government-required charges for use. Additional charges apply depending on services selected, including a carrier Universal Service charge, federal regulatory recovery fee and property tax fee, a one-time High-speed Internet activation fee, a one-time voice service activation fee, extended area service charges, monthly zone increment charges, and connection fees. Taxes, fees, and surcharges apply based on standard monthly, not promotional, rates.

MONTHLY RATE: Monthly rate applies while customer subscribes to all qualifying services. If one or more services are cancelled, the standard monthly rate or rate defined in the tariffs/terms and conditions will apply to each remaining service.

DISCOUNTS AND CREDITS: All applicable discounts and credits will be applied beginning in the first or second full month of billing.

SERVICE AND OFFER-SPECIFIC TERMS: Each section below applies to the specific business service listed. Subscription to specific services or offers by existing customers will cause the loss of any then-current discounts related to such services purchased.

LOCAL SERVICES: Local phone services include direct-dial, local calling, a number of calling features determined by the selected local service plan, and choice of CenturyLink local long distance or toll plans (additional charges apply). Activation fees may apply. Plans exclude dial-up internet connections, conference lines, directory and operator assistance, multi-party conference calls, or other non single path person to person conversation or voice message. CenturyLink reserves the right to move a customer on this plan to an alternative plan if usage on the plan does not resemble typical business, local voice calling.

LONG DISTANCE SERVICES: Per minute and monthly recurring rates apply to one business phone line with direct-dial, nationwide voice calling; including all U.S. states, territories and possessions. Access Line Charge applies to multi-line business customers. Unlimited callings plans exclude data and facsimile services or data and broadcast facsimile services (including dial-up internet connections), conference lines, directory and operator assistance, chat lines, pay-per-call, calling card use, transcriptions, or multi-dwelling units. Restrictions on number of long-distance plans per account or per location apply. International calling available for additional per minute and/or per month charges; all applicable rates are posted to the Website. If a block of minutes is included for long distance calling, the following applies: block minutes do not apply to operator services, switched data services or international calls; usage in carry over to next month; and all International calling is billing separately from block minutes.

HIGH-SPEED INTERNET SERVICES: Speed availability varies by service location. A term commitment (12 months, 24 months, or 36 months) may be required and, if required, an early termination fee of $100 (12-month term), $200 (24-month term) or $300 (36-month term) also will apply. Activation fees may apply. An additional monthly or flat fee, installation fee, and separate shipping and handling fee may apply to customerís modem or router, depending on HSI package and options selected. Connection speeds are based on sync rates. Customers must accept High-Speed Internet Subscriber Agreement or Business-Class High-Speed Internet terms and conditions, as applicable. Listed broadband speeds vary due to conditions outside of network control, including customer location and equipment, and are not guaranteed. Consistent speed claim and private direct connection claim as well as claim that your connection to CenturyLink's network is 100% yours is based on CenturyLink providing HSI subscribers with a dedicated virtual circuit connection to the CenturyLink central office.Telephone landline is part of the service only for the purpose of data traffic transmission/connection and cannot be used for voice traffic transmission, except for 911 services.

CHOICE BUSINESS PHONE PLANS: Feature availability varies. Line installation charges may apply. Discounts available with term agreements in 12, 24 or 36 month terms. Early termination fees will apply in the amounts of $100 for a 12-month agreement, $200 for a 24-month agreement and $300 for a 36-month agreement. Service-specific terms set forth in the local and long distance service paragraphs above also apply.

CHOICE UNLIMITED LONG DISTANCE PLAN: Available only for business customers on up to 10 phone lines per location only. With qualifying local service, discounts available with term agreements in 12, 24 or 36 month terms. Early termination fees will apply in the amounts of $30 (12-month term), $60 (24-month term), and $75 (36-month term). For term agreement, all services must be billed on the same invoice. If CenturyLink determines customer is in violation of restrictions listed above in the long distance service paragraph above, customer will forfeit eligibility for rates under this plan and will be moved to a usage sensitive plan of customer's choice. Service-specific terms set forth in the Local and Long Distance Services paragraphs above also apply.

CORE CONNECT®: Offer available to new high-speed Internet business customers only. Listed bundle rate includes High-Speed Internet from 768kbps up to 20mbps where available, unlimited, direct-dial local and domestic long-distance calling and listed features, and requires a 36-month term commitment (after which the rate reverts to the then-current non-Core Connect standard rate). Activation fees may apply. An additional monthly or flat fee, installation fee, and separate shipping and handling fee may apply to customerís modem or router, depending on HSI package and options selected. An early termination fee will apply in the amount of $600 for a 36-month term, prorated for the number of months remaining in the term. All services must be billed on the same invoice. Core Connect Unlimited Long Distance plan is available only for business customers on up to 10 lines per account. If CenturyLink determines customer is in violation of restrictions listed above in the long distance service paragraph, customer will forfeit eligibility for rates under this plan and service will be suspended/terminated. If customer's Core Connect local or HSI services are terminated either by the customer or CenturyLink, the customer's long distance service will be converted to the Choice Unlimited long distance calling plan. Service-specific terms set forth in the Local, Long Distance, and High-Speed Internet Services paragraphs above also apply. $10 off Core Connect: One $10 credit issued each month for 12 months beginning with the first or second invoice.

CORE CONNECT® Professional: Listed bundle rate includes High-Speed Internet service at up to 12mbps, up to 3 business phone lines with unlimited, direct-dial local and domestic long-distance calling, listed features and service packs, and requires a 36-month term commitment (after which the rate reverts to the then-current non-Core Connect standard rate). Activation fees may apply. An additional monthly or flat fee, installation fee, and separate shipping and handling fee may apply to customerís modem or router, depending on HSI package and options selected. An early termination fee will apply in the amount of $600 for a 36-month term pro-rated for the number of months remaining in the term. All services must be billed on the same invoice. Core Connect Professional Unlimited Long Distance plan is available only for business customers on up 10 lines per account. If CenturyLink determines customer is in violation of restrictions listed above in the long distance service paragraph, customer shall forfeit eligibility for rates under this plan and service will be suspended/terminated. If customer's Core Connect local or HSI services are terminated either by the customer or CenturyLink, the customer's long distance service will be converted to the Choice Unlimited long distance calling plan. Service-specific terms set forth in the Local, Long Distance, and High-Speed Internet Services paragraphs above also apply.

CORE CONNECT®; PROFESSIONAL AND CENTURYLINK FIBER+ DATA AND VOICE NETWORK AVAILABILITY SERVICE LEVEL AGREEMENT (SLA) - SLA provides 99.9% CenturyLink Data and Voice network availability. CenturyLink Data and Voice network availability is the ability to transmit data or voice over the CenturyLink network. This SLA does not apply to unavailability caused by customer premise equipment (including viruses or damage to equipment), reductions in or degradation of bandwidth or speed, international or operator assisted calls, or any event outside CenturyLink's control. Offer requires customer to remain in good standing and terminates if customer cancels Core Connect Professional or CenturyLink Fiber+. To qualify for credit, customer must report any qualifying event to CenturyLink by calling the CenturyLink Customer toll free number within 24 hours of the event. CenturyLink will credit qualifying customers: (1) 50% of the Core Connect Professional or CenturyLink Fiber+ monthly recurring charge ("MRC") (less applicable discounts) if CenturyLink data or voice network is unavailable between 43 and 432 consecutive minutes (7.2 consecutive hours) in a single billing period; or (2) 100% of the Core Connect Professional or CenturyLink Fiber+ MRC (less applicable discounts) if CenturyLink data or voice network is unavailable more than 432 consecutive minutes (7.2 consecutive hours) in a single billing period. Regardless of the number of qualifying events, no customer will receive, in a single billing period, more than 100% of the Core Connect Professional or CenturyLink Fiber+ MRC per bundle. Credit(s) are applied to subsequent month's bill. The total credits given during a 12-month period shall not exceed the value of two full MRC's. If customer experiences and reports qualified events and credit requests exceed allowed credits within a single billing period or within a 12-month period, customer will be allowed, upon customer's request, to terminate Core Connect Professional or CenturyLink Fiber+ service without paying an early termination fee.

HIGH-SPEED INTERNET (1.5mbps) WITH UNLIMITED LOCAL DOUBLE PLAY OFFER: Listed rate available to new High-Speed Internet business customers subscribing to 1.5mbps speed tier with Office Plus and unlimited local calling. 36-month term commitment required (after 12 months the rate reverts to the then-current standard rate.) An early termination fee will apply in the amount of $300 for the 36-month term. Service-specific terms set forth in the Local and High-Speed Internet Services paragraphs above also apply.

ENHANCED DOUBLE PLAY BUNDLE OFFER: New high-speed Internet business customers only. Suspected misuse or abuse of any website maintenance support may result in termination of support. Customer may incur additional costs for additional maintenance time over the included 30 minutes per month. Early termination fees: If customer terminates before end of term, customer will receive a full refund for termination within 30 days from order date if no pre-development work has begun. If pre-development work has begun, and customer is still within the first 30 days after order date, customer will receive a 50% refund. For early termination after 30 days or after development has begun, an early termination charge equal to the monthly recurring charge for any months remaining in the term, plus any charges for services accrued but not yet paid will apply.

30-Day Risk-Free Trial: Available to new High-Speed Internet subscribers who call CenturyLink to cancel within first 30 days of service following High-Speed Internet service activation. Refund excludes selected optional charges, such as professional installation. Cancellations within first 30 days following High-Speed Internet service activation will not incur early termination fee. Additional monthly recurring service charge to upgrade current HSI speed will be billed to customer. Excludes taxes, fees and surcharges. Features may be added, modified, or removed during the service period and customer should install updates when they become available. Office 365 is designed to work with the current or immediately previous version of Internet Explorer or Firefox, or the latest version of Chrome or Safari.

SAAS CLOUD APPLICATION FIRST MONTH FREE OFFER: Offer available to new or existing customers making a new Software-as-a-Service (SaaS) purchase, who do not already host their website with CenturyLink. Available on qualifying SaaS applications except DNR/T and SSL Certificate.
