GENERAL TERMS:

Offers and rates apply to business customers only. The following sections apply to all business services: General, Terms and Conditions, Taxes, Fees and Surcharges, Monthly Rate, and Discounts and Credits.

GENERAL: Services and offers not available everywhere. CenturyLink may change or cancel services, or substitute similar services at its sole discretion without notice. Requires credit approval and deposit may be required. Additional restrictions apply.

TERMS AND CONDITIONS: All products and services are governed by tariffs, terms of service, or terms and conditions posted at http://www.centurylink.com/Pages/AboutUs/Legal/ ("Website"). Products and services are provided by the CenturyLink operating company serving your service location.

TAXES, FEES, AND SURCHARGES: All prices exclude taxes, fees and surcharges, which apply to all services, and include federal and state-mandated and permitted charges, cost recovery charges, state and local fees that vary by area, and certain in-state surcharges. Cost recovery fees are not taxes or government-required charges for use. Additional charges apply depending on services selected, including a carrier Universal Service charge, federal regulatory recovery fee and property tax fee, a one-time High-speed Internet activation fee, a one-time voice service activation fee, extended area service charges, monthly zone increment charges, and connection fees. Taxes, fees, and surcharges apply based on standard monthly, not promotional, rates.

MONTHLY RATE: Monthly rate applies while customer subscribes to all qualifying services. If one or more services are cancelled, the standard monthly rate or rate defined in the tariffs/terms and conditions will apply to each remaining service.

DISCOUNTS AND CREDITS: All applicable discounts and credits will be applied beginning in the first or second full month of billing.


SERVICE TERMS:

Each section below applies to the specific business service listed. Subscription to specific services or offers by existing customers will cause the loss of any then-current discounts related to such services purchased.


BUSINESS PHONE TERMS

LOCAL SERVICES: Local phone services include direct-dial, local calling, a number of calling features determined by the selected local service plan, and choice of CenturyLink local long distance or toll plans (additional charges apply). Activation fees may apply. Plans exclude dial-up internet connections, conference lines, directory and operator assistance, multi-party conference calls, or other non single path person to person conversation or voice message. CenturyLink reserves the right to move a customer on this plan to an alternative plan if usage on the plan does not resemble typical business, local voice calling.

LONG DISTANCE SERVICES: Per minute and monthly recurring rates apply to one business phone line with direct-dial, nationwide voice calling; including all U.S. states, territories and possessions. Access Line Charge applies to multi-line business customers. Unlimited callings plans exclude data and facsimile services or data and broadcast facsimile services (including dial-up internet connections), conference lines, directory and operator assistance, chat lines, pay-per-call, calling card use, transcriptions, or multi-dwelling units. Restrictions on number of long-distance plans per account or per location apply. International calling available for additional per minute and/or per month charges; all applicable rates are posted to the Website. If a block of minutes is included for long distance calling, the following applies: block minutes do not apply to operator services, switched data services or international calls; usage in carry over to next month; and all International calling is billing separately from block minutes.


BUSINESS INTERNET AND DATA TERMS

HIGH-SPEED INTERNET SERVICES: Speed availability varies by service location. A term commitment may be required and, if required, an early termination fee $200 will apply. Activation fees may apply. An additional monthly or flat fee, installation fee, and separate shipping and handling fee may apply to customerís modem or router, depending on High Speed Internet package and options selected. Connection speeds are based on sync rates. Customers must accept High-Speed Internet Subscriber Agreement or Business-Class High-Speed Internet terms and conditions, as applicable. Listed broadband speeds vary due to conditions outside of network control, including customer location and equipment, and are not guaranteed. Consistent speed claim and private direct connection claim is based on CenturyLink providing HSI subscribers with a dedicated virtual circuit connection to the CenturyLink central office.


BUSINESS SOLUTIONS TERMS

MICROSOFT OFFICE 365: Customer will receive subscriptions based on the CenturyLink bundle purchased to use the products on the specified number of PCs during customerís subscription term with CenturyLink, which begins upon initial installation and activation of the product. These services are subject to acceptance of the terms and conditions included with the product. Features may be added, modified, or removed during the service period and customer should install updates when they become available. Office 365 is designed to work with the current or immediately previous version of Internet Explorer or Firefox, or the latest version of Chrome or Safari.


PROMOTIONAL OFFER TERMS

Pure Business Internet: Listed rate includes High-Speed Internet and requires a 24-month-term commitment and enrollment in CenturyLink AutoPay and electronic billing service within the specified promotional timeframe to be eligible for rate. An additional monthly or flat fee, installation fee, and separate shipping and handling fee may apply to customerís modem or router, depending on HSI package and options selected. A $200 fee prorated for the number of month remaining in the term will apply to early termination of the High-Speed Internet service. Telephone landline is part of the service only for the purpose of data traffic transmission/connection and cannot be used for voice traffic transmission, except for 911 services. Service-specific terms set forth in the High-Speed Internet Services paragraph above also apply.


BUSINESS SOLUTIONS OFFER TERMS

CENTURYLINK® BUSINESS BUNDLES: Listed bundle rate includes High-Speed Internet, unlimited local and domestic long-distance calling and listed features, and requires a 24-month term commitment and enrollment in CenturyLink AutoPay and electronic billing service within the specified promotional timeframe to be eligible for rate. An additional monthly or flat fee, installation fee, and separate shipping and handling fee may apply to customer's modem or router, depending on HSI package and options selected. If one (1) or more services are terminated before the end of the required term commitment, services may be converted to standalone plans, the standard monthly fee will apply to each remaining service and a $400 fee prorated for the number of months remaining in the term will apply. Service-specific terms set forth in the Local, Long Distance, and High-Speed Internet Services paragraphs above also apply.

SAAS CLOUD APPLICATION FIRST MONTH FREE OFFER: Offer available to new or existing customers making a new Software-as-a-Service (SaaS) purchase, who do not already host their website with CenturyLink. Available on qualifying SaaS applications except DNR/T and SSL Certificate. Customer may need to input valid credit card number with U.S. based billing address and qualifying promo code at time of purchase. Customer will be billed the monthly service charge 30 days after purchase and each month until service is canceled by customer. Monthly recurring charges will be assessed in advance and are non-refundable under any circumstances and usage-based and non-recurring charges will be assessed when incurred or on the first day of the next billing cycle, at CenturyLinkís sole discretion. Customer must accept CenturyLinkís terms and conditions at https://apps.centurylink.com/terms-conditions and all applicable third party terms and conditions, as required. Service level agreements (SLAs) in the terms and conditions describe customerís sole remedy for service quality or performance issues of any kind. Monthly recurring charges include listed features and service may have a bundle term commitment with an early termination fee.

TRIPLE PLAY BUSINESS BUNDLE WITH DIRECTV OFFER: The listed bundle monthly rate includes high-speed Internet up to 7Mbps depending on availability, unlimited direct-dial local and domestic long distance service, and DIRECTV Business Information service package and requires a 24-month term agreement for all services including Autopay and electronic billing enrollment within the specified promotional timeframe (after 12 months the rate reverts to the then-current standard rate). Activation fees may apply. An additional monthly or flat fee, installation fee, and separate shipping and handling fee may apply to customer's modem or router, depending on High Speed Internet package and options selected. If one (1) or more services are terminated, the standard monthly fee will apply to each remaining service and a $400 early termination fee will apply prorated for the number of months remaining in the term. Service-specific terms set forth in the Local, Long Distance, High-Speed Internet and CenturyLink Business Bundle Services paragraphs above and the DIRECTV offer paragraphs also apply.


OTHER PROMOTIONAL OFFER TERMS

Price Lock / Fixed-Rate Pricing - Applies only to the monthly recurring charges for the 24-month term for the listed services; excludes all taxes, fees and surcharges, monthly recurring fees for modem/router and professional installation, and shipping and handling fee for customer's modem or router. Offer requires customer to remain in good standing and terminates if customer changes its account in any manner, including any change to the required CenturyLink services (canceled, upgraded, downgraded), telephone number change, or change of physical location of any installed service (including customer moves from location of installed services).
