Name
Indicates the name of the person using the digital certificate. This field has two parts, separated by a space.
Password
Enter your password for logging into the system.
Retype Password
Reenter the password for logging into the system.
Phone
Indicates the Telephone number of the person assigned to the digital certificate.
Indicates the Email address of the person assigned to the digital certificate.
Email1
Indicates the Email address of any other person you want to be notified.
Email2
Indicates the Email address of any other person you want to be notified.
Fax
Indicates the fax number of the person assigned to the digital certificate.
Submit
Click this button to submit the details entered on screen.
Clear
Click this button to clear all the details on screen.
