
Number
Indicates the street number of the street address you would like to validate. For example: 100 Walnut Av. or 2913 Mickelson Blvd.
You can enter a maximum of 10 alphanumeric characters, including hyphen (-). The hyphen (-) is used to indicate fractional street numbers. For example, 34-1/2 West Third Street. The hyphen (-) cannot be the first character.
Street Name
Indicates the name of the street address you would like to validate. For example, 100 Walnut Av. or 2913 Mickelson Blvd.
You can enter a maximum of 30 alphanumeric characters, including hyphen (-) and apostrophe ('). The hyphen (-) is used to indicate hyphenated names and fractional street names. for example, County Rd 2-1/2. The apostrophe is used, when applicable, in street names. For example, O'Neil Av.
You can enter the street name in upper or lower case.
When an address contains a direction such as East, South, Northeast, refer to the rules and abbreviations documented in the section Directional Abbreviations.
City
Indicates the name of the city where the address you would like to validate is located. For example, Denver, Seattle. You can enter a maximum of 32 alphanumeric characters.
Enter the full name of the city in upper or lower case.
State
Indicates the standard abbreviation of the state where the address you would like to validate is located. For example, Colorado- CO or Washington - WA. You can enter a maximum of two alphabetic characters.
For your convenience, a drop-down list of the state abbreviations in the Qwest serving region is provided. You can either select a state abbreviation from this list or enter the first character of the abbreviation in the field.
You cannot enter a
state abbreviation for a state that is not in the Qwest region. If you
try to enter another abbreviation, the Qwest CEMR application will fill
one of the valid abbreviations that contain the same letters, if any.
Description
The description of your search criteria. You can enter a maximum of 60 alphanumeric characters.
Street Type
Indicates the abbreviation for the street type in which the address you want to validate is located. For example, 100 Walnut Av or 2913 Mickelson Blvd. For your convenience, a drop-down list of the valid street type abbreviations used by CEMR is provided. Select a street type abbreviation from this list. Refer to the rules and abbreviations documented in the section Directional Abbreviations.
If the street name
is a numeral greater than nine (such as 212 157th St), do not use the
"St" abbreviation in the Street Type field: Leave the field
blank.
Apartment
Indicates the apartment, suite, room, unit, slip, or lot in which the address you want to validate is located. For example, Apartment: 10B or Room: 121E. You can enter a maximum of four alphanumeric characters.
Building
Indicates the building, wing, or pier where the address you want to validate is located. For example, Building: 4 or Pier: 37. You can enter a maximum of four alphanumeric characters.
Floor
Indicates the floor where the address you want to validate is located. For example, Floor: 11. You can enter a maximum of four alphanumeric characters.
Zip
The zip code of the area. This field must be in either of the following formats:
* Five digits. For example, 12345
