The approved Simple Port Request validation fields include:

AN - Required (CenturyLink includes the 3 digit CUS Code as part of a valid Account Number)

Ported NBR - Required (10 digit Number to be Ported)

Zip Code (Not used by CenturyLink)

Passcode (Not used by CenturyLink)

NOTE: Keep in mind that Required fields for accurately Provisioning of the request are; CCNA, PON, REQTYP, SPDDDT, CC, TEL NO, and the new End User Listing Treatment (ELT) field.
