Customer Questionnaires - V57.0
To do business with CenturyLink™ as a Facility-Based Competitive Local Exchange Carrier (CLEC) or Reseller, the Customer Questionnaire must be completed by you or a representative from your organization. Your CenturyLink Service Manager will assist you with this process.
The Customer Questionnaire contains instructions outlining what sections must be completed before you can begin to order products and/or services.
The on-line Questionnaire, via the web interface, requires the initial Questionnaire to be completed in its entirety, all sections, before it is submitted and accepted. Once the Questionnaire has been completed and accepted, you will have the ability to update, change, or correct specific sections at anytime via the web interface. The Questionnaire instructs you to complete some additional documents within two weeks after submitting the Questionnaire. If you fail to complete these documents, it could delay your ability to order products and/or services as you may not have access to various tools and reports within CenturyLink.
Upon completion of the additional documents, they should be e-mailed to firstname.lastname@example.org. Wholesale will provide you an e-mail response that your documents have been received and are in process.
CenturyLink requires you update your questionnaire(s) on either an "as needed basis" or yearly to reflect any changes within your company that will assist CenturyLink in providing you service.
The completed questionnaire is used by CenturyLink to determine the following:
- Determine geographical requirements
- Assign Internal CenturyLink CLEC or Reseller IDs and Identify Industry Codes
- Determine CenturyLink system requirements to support CLEC specific activity
- Obtain credit information
- Obtain service volume estimates
- Determine deposits, when applicable
- Obtain billing information
- Create Summary Bills
- Obtain Performance Assurance Plan (PAP) information
- Establish output and input requirements
- Obtain your contacts hours-of-operation and holiday schedule
