Change to the DLIS system, per specifications below:

The Directory Listing Inquiry System (DLIS) will now be accessed via the following URL: https://kai03.qwest.com/Qsearch/DLIS/Login.aspx

DLIS will be modified to expand the listing data returned and enhance the user experience.

1.) The Directory Listing Detail Response Screen will be changed to:

? Segment data by: Account Information, Service Address, Directory Listing Information, Prior Level Information and Delivery Address.

? Service Address fields are parsed (SAPR, SANO, SASF, SASD, SASN, SATH, SASS, LD1/LV1-LD3/LV3.

? Directory Listing Information fields are included and parsed. Fields included are: LTN/NSTN, OMTN, COMPANY CODE, ALI, RTY, LTY, STYC, TOA, DOI, DML, NOSL, TMKT, BRO, DIRNAME, LNPL, LNLN, LNFN, DES, TL, TITLE1, NICK, PLA, ADI, LAPR, LANO, LASF, LASD, LASN, LATH, LASS, LOCATION TEXT, LALOC, LAST, HTN, HNSTN, SO, LTXTY, LTEXT.

? Prior Level Information fields are included: LVL, PLINFO, PLTN, PLNSTN, SO

? Delivery Address fields are included: DIRQTYA, DIRQTYNC, DDAPR, DDANO, DDASF, DDASD, DDASN, DDATH, DDASS, LD1/LV1-LD3/LV3, CITY, STATE and ZIP.

2.) A new History tab will be added to the Directory Listing Detail Response Screen. The Listing History Display Screen includes the following fields: Account Number, Listed Telephone Number, Order Number, Activity Type, and Completion Date.

3.) A new LTN Display Search Response will be added when multiple listings are returned. The following fields are included: LNLN, LNFN, NSTN, ALI, SO, TOA, RTY, City, State and WPD.

4.) A new Account Number Search functionality will be added.

5.) A new AN Display Search Response will be added when multiple ANs are returned. The following fields are included: AN, LTN and Provider ID.

6.) A History tab will be added to the AN Display Search Response Screen which includes the following fields: Account Number, Company Code, Order Number, Activity Type, and Completion Date.

7.) A new SLU Display Search Response will be added when multiple listings are returned. The

following fields are included: SLU/List Name Text, LTN, City, State and WPD.

8.) A new NSTN Display Search Response will be added when multiple listings are returned. The following fields are included: LNLN, LNFN, NSTN, ALI, SO, TOA, RTY, City, State and WPD.

9.) City and State will no longer be required for a Service Access Code (SAC) search (800 type numbers).

10.) A new SAC Display Search Response will be added when multiple listings are returned. The following fields are included: LNLN, LNFN, SAC, ALI, SO, TOA, RTY, City, State and WPD.

11.) Error Code Matrix will be updated.

12.) DLIS User Guide will be updated.

Connie Winston/Qwest stated that this request impacts the Directory Listing Inquiry Systems (DLIS). Connie said that we are making some screen improvements that will make the GUI more usable. Connie stated that we are targeting a February 27, 2005 implementation date. Bonnie Johnson/Eschelon asked if the CLECs needed to do any coding changes. Connie Winston/Qwest said no. She also said that we have been monitoring the DLIS usage and that the CLECs are still using it. Jill Martain/Qwest stated that the status of this CR will change to Presented.

RE: SCR011205-01 Changes to the DLIS System to enhance the customer experience.

Qwest has reviewed the information submitted as part of Change Request (SCR020205-01). Based upon the scope of this CR as agreed to in the Clarification Meeting (held January 13, 2005) Qwest is able to provide an estimated Level of Effort (LOE) of 720 to 960 hours for this DLIS Change Request.

At the next Monthly Systems CMP Meeting, CMP participants will be given the opportunity to comment on this Change Request and provide additional clarifications. Any clarifications and/or modifications identified at that time will be incorporated into Qwest's further evaluation of this Change Request.
