CustomChoice® for Business Package allows end-users to choose an unlimited line with features from a menu of Custom Calling, Custom Local Area Signaling Services (CLASS™) and other features for a flat monthly rate. CustomChoice for Business Package end-users are also eligible for discounted rates on other products where these products are available.
Qwest Utility Line provides an additional line. This line can only be added in conjunction with a contracted CustomChoice for Business Package. The Qwest Utility Line can only be provided on a one-to-one basis (maximum of one Qwest Utility Line with each CustomChoice for Business package). No additional services, such as features or listings, are allowed with this line.
Contracts do not have to be signed, but Termination Liability Assessment (TLA) may apply if a contracted package is removed prior to the expiration of the contract term.

CustomChoice for Business Package has a recurring charge for the package, except in ID-S that bills the difference between the line rate and the package rate. There are no nonrecurring charges for the features included in the package. There may be a line nonrecurring charge. Recurring and nonrecurring charges are based on state specific Tariffs/Catalogs/Price Lists.

Two-Line Packages
When adding, changing, removing or converting 2-Line CustomChoice for Business, you must provide the USOC PGO2N. PGO2N is used to identify the second line in a two-line package it is not used for billing or provisioning the line; it enables Qwest to correctly identify and bill the package elements on the account. The following ordering rules illustrate examples of when the USOC is required.
