Commercial Solutions Customer Questionnaires - V7.0

Following negotiations with CenturyLink and after you have completed the applicable Interconnection Agreement or Amendment, and/or Commercial Agreements, you must complete a Customer Questionnaire(s). The questionnaire(s) is used by CenturyLink to determine the following:

  • Geographical requirements
  • CLEC IDs
  • System requirements to support CLEC specific activity
  • Credit information
  • Service Volume estimates
  • Deposit requirements
  • Billing and Summary Bill information
  • Output and input requirements
  • CLEC contact information

The Questionnaire(s) contains instructions outlining what sections must be completed. Your CenturyLink Service Manager is available to assist you with this process.

If the Customer Questionnaire(s) is filled out incorrectly and/or incompletely, it will be returned to you via email for completion and/or correction.

Note that the Questionnaire(s) instructs you to complete certain additional sections within two weeks after submitting the initial required sections to CenturyLink. If you do not submit these additional sections, product availability may be delayed. E-mail these additional sections to wholesale.servicesupportteam@centurylink.com.

The Customer Questionnaire includes the information needed for Commercial Solutions. You can access the Customer CLEC Questionnaire for Facility-Based CLEC from http://questionnaire.CenturyLinkapps.com.
