Emergency Broadband Benefit Program

For CenturyLink to apply the Emergency Broadband Benefit discount to your account, you must fill out the form below. Before filling out this form, please complete the following steps:
  1. Apply for the Emergency Broadband Benefit program at https://getemergencybroadband.org/. Please use the information for the primary CenturyLink account holder when filling out the program application.
  2. Receive confirmation on your eligibility for the Emergency Broadband Benefit from the National Verifier.
  3. If you aren't currently a customer, please sign up for CenturyLink Internet service at https://www.centurylink.com/internet/.
Once you submit the form, CenturyLink will:
  • Verify your eligibility for the Emergency Broadband Benefit program.
  • Verify your CenturyLink account.
  • Send an email notification with the status of your application. Be sure to review this email as there may be additional action required.
  • Apply the Emergency Broadband Benefit discount to your CenturyLink account within one billing cycle.
How do I find my CenturyLink Account Number?

You can find your account number on the first page of your CenturyLink bill in the very top row. If you don't have a paper bill handy, you can try retrieving your account number via email address.

The Emergency Broadband Benefit Program is a temporary program of the federal government that reduces your broadband Internet service bill. When the Emergency Broadband Benefit Program ends, you may continue to receive Internet service from CenturyLink, and will be subject to the general rates, terms, and conditions applicable to the service you have subscribed to.
You may obtain broadband Internet service supported by the Emergency Broadband Benefit Program from any participating provider of your choosing, and you may transfer your benefit to another provider at any time.

When the Emergency Broadband Benefit Program ends, you can choose to continue to receive Internet service, subject to the generally applicable rates, terms, and conditions for your service, or you can choose to have your Internet service end automatically. You may change your mind and make a different selection any time before the program ends. Please select one of the following options:
I want to continue to receive Internet service from CenturyLink once the Emergency Broadband Benefit Program is ended by the federal government.
I want to participate in the Emergency Broadband Benefit Program, but do not want to continue to receive internet service from CenturyLink once the Emergency Broadband Benefit program has ended. Consent to Transmit Information to Emergency Broadband Benefit Program Administrator
To enroll you in the Emergency Broadband Benefit Program, we must transmit the following information about your enrollment to the administrator of the Program, the Universal Service Administrative Company (USAC) to ensure the proper administration of the program: your full name, full address, date of birth, last four digits of your Social Security Number (if applicable), and telephone number associated with your enrollment. If you consent to us transmitting this information, please indicate so by checking below. If you do not consent, we will not be able to enroll you in the program and you will not receive the benefit.
